Re: Chapter Recognition, Awards

I greet you in the strong name of Omega. Our district meeting is vastly approaching and now is the time to recognize those chapters for their exemplary progress and activities for the previous year. 

Listed below are the three awards that will be given for your accomplishments:

·         Chapter of the Year

·         Community Service Award

·         Best Scrapbook

 The awards will be presented to an undergraduate and graduate chapter in each category that meets the criteria and qualifications.

 For your convenience I have included instructions for all submissions. Highlight your activities for the fiscal year 2003/2004. All information must be received by February 28, 2005.

Instructions for all Submissions

The goals of this committee are to recognize outstanding achievement in community and civic affairs for the year.

 The objectives are to critique the Graduate and Undergraduate activities and award those chapters that submit their data in the following categories. 

o        Chapter of the Year

o        Community Service Award

o        Best Scrapbook

 Items needed

  1. Attach and complete Form-37

  2. Attach and complete Form-53

  3. Attach and complete Form-50

 

* All forms can be downloaded from the Omega Psi Phi Fraternity, Inc. Official Website.

This year more emphasis will be placed on the completion of Form-37. You will be required to provide supported documentation. There are eight sections on the Form-37. Listed below are the attachments that will be needed.

Chapter Membership:

  1. Name of Financial Brothers – Attach Form-53

  2. Number of Brothers Reinstated – List each brothers name and control number (#)

  3. Number of Brothers initiated – List each brothers name and control (#)

  4. Number of Brothers who have moved – List each brothers name and control  (#)

  5. Number of inactive brothers – Total Count

  6. Number of Brothers who have died – List names

  7. List name of delegate(s): Conclave and District Meeting.

  8. Attach Form-50

 

* Please list the total number of brothers and chapter officers that attended the Conclave, District Meeting, and Leadership Meeting.

 Chapter Scholarships, Gifts and Contributions:

1.       Total number of Scholarships awarded – List name of each recipient, school attended, amount of scholarship.

2.        Contributions: List estimated man hours spent and actual dollar amount given.

* Undergraduate chapter provide name and current address of all brothers who graduated last year.

 

For consideration your chapter must be in good standing with International Headquarters and the District. Submissions must be received by Monday, February 28,2005. The Community and Civic Affairs Committee will serve as the body of judges that will select the winners.

 *Scrapbook participants must have their books on display:

  Friday, March 25, 2005 from 8:00a.m – 5:00p.m.

 Saturday, March 26,2005 from 8:00a.m. - 1:00 p.m.

 

The committee assumes no responsibility for books on display during the time periods. Judging will take place on Saturday, March 26, 2005
